Physical Examination Requirements

According to New York State Education Law, a physical examination which includes the new Body Mass Index (BMI) screening is required for all new entrants to the school, pre-kindergarten, kindergarten and grades 1, 3, 5, 7, 9 and 11. The BMI/Weight Status Category will be included on the mandated survey required by New York State. Parents may opt to have their child’s BMI/Weight Status excluded from this survey by sending a written request to the Health Office. 
 
These physicals should be performed by your own physician as each child has the best assurance of proper attention to his or her health needs when examined by his or her own doctor. The school district encourages this practice. A physical examination form will be mailed home. You may also print a copy by clicking on the file at right. Please have your physician complete the form and return it to the school nurse prior to September 1 of the current school year. 
 
If you plan to have your child’s physical evaluation performed by your private physician, you must notify the Health Office or return the required physical forms to the Health Office by September 30 of each school year. If no notification is received it is assumed that you wish your child to receive a health appraisal by the school physician.
