Dental Certificate Legislation
Since September 1, 2008, the New York State Department of Health is requiring that school districts request Dental Health Certificates for new entrants, pre-kindergarten, kindergarten, grades 1, 3, 5, 7 and 11. Dental Health Certificate forms will be mailed with the required physical forms, you may pick one up in the Health Office, or you may print a copy by clicking on the file at right. The Health Office has a list of participating dentists that will provide free or reduced dental screenings.
