Health Information Privacy
The District and its employees, defined as a “covered entity,” comply with the Health Insurance Portability and Accountability Act (HIPPA) and will require parent authorization to disclose or discuss protected health information. Please contact your child’s school nurse or the Office of Special Programs to obtain a copy of the Holley Central School District HIPPA Authorization form.
